Vision Plan
The University of Alaska partners with VSP to provide ÂÌÅ«Ì컨°å employees with a vision plan.
Vision Plan Enrollment
- Benefit-eligible employees working a minimum of 20 hours per week
- Temporary employees who meet the hours worked requirement (check with or visit the temporary employee webpage)
- Eligible if enrolled in same plan as the employee

- 30 day deadline from hire date to enroll or opt out
- If no form submitted, defaulted into Copay medical, Basic dental, and vision for employee-only coverage
- Review the qualifying life events page
- 30 day deadline from life event (60 days for birth or adoption)
- Late forms not accepted

- Submit form by 5:00pm on the Thursday prior to the end of their first pay period
- If submitted later but within the 30 day requirement, coverage begins on the first day of the pay period the form is submitted
- Submit form on or before the day of the event
- If submitted after the day of the event but within the 30 day requirement (60 for birth/adoption), coverage begins on the day the forms is submitted
- End of the pay period in which an employee (1) separates from the University or (2) ends coverage due to a life event

Vision Plan Details​
Use ÂÌÅ«Ì컨°å ID with leading zero (i.e. 030011234)
- Employees and all covered dependents use the same member ID
- VSP does not mail cards; use ÂÌÅ«Ì컨°å ID with leading zero at point of service
- Do not use your SSN for VSP benefits

$10 copay
The Eye Guys travel around the state of Alaska to provide preventive eye exams at many rural locations. Review on ongoing news article to see where The Eye Guys will be for this month.

Up to $39 copay, coverage not available at retail chains
As medically necessary
$20 copay per office visit
- Members with a diagnosis of diabetes receive a covered-in-full retinal screening
- Coordination with medical coverage may apply
- Services include
- Pink eye treatment
- Sudden changes in vision
- Monitor ongoing conditions such as dry eye
- Diabetic eye disease
- Glaucoma, etc.

- Review both the "lenses" and "frames" drop-down for information on the allowances every other year

- Single vision, lined bifocal, lined trifocal are all included with the prescription glasses and available every other plan year after the $25 copay

- Eyemart Express is a leading national optical retailer that joined the VSP network as a Premier Edge retail location
- To find an Eyemart Express location near you, log into your account on and use the "Find a Doctor" search
- Eyemart Express will appear as an in-network provider for applicable members
- There is one Eyemart Express location in Anchorage and one in Fairbanks
- There are multiple Premier Edge locations throughout the state
- Use the vsp.com "Find a Doctor" search to locate these retailers
Online prescription and non-prescription eye wear
allows VSP members to apply their benefits toward purchasing prescription and non-prescription eye wear, including popular contact lens brands at competitive prices.
